On your first visit to our office, you will be given a thorough examination by one of our doctors, which may include a panoramic X-ray and photos. The doctor will discuss your orthodontic needs and recommend treatment options. Our treatment coordinator will be able to discuss the cost of all services and, if you’re interested, a flexible payment plan. We will answer any questions that you may have at this time. Our primary goal is to help our patients feel confident that they will receive the best and most appropriate treatment at our office.
For your convenience, you may print and bring these completed forms with you on your first visit.

It will also be helpful for you to bring your dental insurance information at this time.
If your child is not quite ready for treatment, we can schedule growth checks for your child. This will allow us to monitor your child’s growth and to determine when and if your child will benefit from treatment.
